GLP-1 medications dramatically reduce appetite — but they don't guarantee optimal nutrition. When you're eating significantly less, what you eat becomes more important than ever. Here's how to structure your diet on GLP-1 therapy for maximum results and minimum muscle loss.
The Core Challenge
GLP-1 medications can reduce calorie intake by 30–50%. At 1200–1500 calories per day, meeting protein, micronutrient, and fiber targets is genuinely difficult. Many patients on GLP-1 drugs lose significant muscle mass alongside fat — partly due to inadequate protein intake at very low calorie intakes.
Protein First, Every Meal
With reduced appetite, every bite counts. Prioritize protein-dense foods at every meal — eggs, Greek yogurt, cottage cheese, chicken, fish, or protein shakes. Targeting 1.6–2g/kg bodyweight is still the goal, even (especially) on GLP-1 medications. If appetite suppression makes this difficult, liquid protein sources (shakes, Greek yogurt) are more digestible.
Foods to Avoid
High-fat, greasy, or fried foods dramatically worsen GLP-1 nausea. Carbonated drinks, large portions, and eating too quickly all increase GI side effects. Very sugary or high-glycemic foods cause pronounced blood sugar swings that worsen nausea. Small, frequent, protein-forward meals typically minimize side effects.
Foods That Work Well
Lean proteins, non-starchy vegetables, eggs, Greek yogurt, soft foods (smoothies, soups, oatmeal), and easily digestible carbohydrates (rice, sweet potato) tend to be well-tolerated. The goal is nutrient density per calorie — when appetite is suppressed, empty calories are a significant opportunity cost.
Resistance Training is Non-Negotiable
Diet alone cannot preserve muscle on the scale of caloric restriction typical with GLP-1 drugs. Resistance training 2–3x per week, combined with adequate protein, is essential for maintaining lean mass and metabolic rate. Patients who combine GLP-1 therapy with resistance training and protein optimization lose a significantly higher proportion of fat versus muscle.
GLP-1 drugs are a powerful tool — but maximizing outcomes requires optimizing what you eat within the reduced appetite framework. Protein, resistance training, and nutrient density are the three pillars that separate good outcomes from great ones.
Six months on Wegovy and this is the most practical dietary guide I've found. The nausea from greasy food is very real — avoid at all costs.
